Patient Registration
Dr. Chamberland and our team are committed to providing you with a convenient dental experience that respects your time and busy schedule. To help you save time at our office and see our dentist quicker, we invite new and returning patients to complete their patient forms in advance before visiting our dental practice. We encourage you to contact Chamberland Dentistry if you have any questions or would like to schedule an appointment.
If you are new to our practice, we will need to gather some information from you. New patient and update forms are available here on our website. You will be asked to update once a year.
For patients of majority age, fill out the Patient Registration, Medical History and Patient Consent for an Adult forms. Patients that are minors (under the age of 18), fill out the Patient Registration, Medical History and Patient Consent for a Minor forms. All minors will need to be accompanied by a parent; preferably, or a legal guardian by executed legal document (ex: POA), we have a Limited POA form available for your convenience that will need to be executed prior to the day of the appointment.

Appointments
For your first appointment, please also bring a government issued photo ID and your insurance card if you have insurance. Please also remember to bring a list of any medications that you are currently taking, including any over the counter medications. Also, should you have recent X-rays please have them sent to our office via email to: office@chamberlanddentistry.com.
Please present your insurance cards to the receptionist before being seen. If at any time your insurance coverage changes, or if there are any address changes, please contact our office with that information.
Filing insurance is done as a courtesy, all balances, (out of pocket due from patient) not paid by the insurance company are due at time of service.
Please check your handbook or call your insurance company if there are questions regarding benefits or physician participation in your insurance network.
If you are unable to keep a scheduled appointment, we ask that you please give us 24 hours’ notice so that we may give your appointment to someone in need.
Prescription Refills
For prescription refill, please contact us with the following information:

- Name of Prescription & Dosage
Please allow 48 hours to process prescription refill requests. As a general policy, we do not process request refills after hours or on weekends.
